How to upload documentation of immunizations & medical records

 Freshman students:

  • If you are a freshman student seeking to upload documentation, you must wait until you receive a LinkBlue ID so you can access the Student Health Patient Portal. 
  • If you need to send records before you get your LinkBlue ID, you may email records to  
  • If you are having difficulty accessing the system, please contact

College of Dentistry, College of Public Health and College of Pharmacy students in clinical rotations: 

  • Complete the Health Science Compliance Information form found on the patient portal under forms.
  • Upload your documentation (immunization records, titer results, etc.).
  • Please allow 1-2 weeks for the nurse to review your records. 

College of Medicine, College of Nursing and College of Health Sciences:

  • Send your immunization records directly to Castle Branch as instructed by your respective program.  
  • How to upload documentation

    1. Log in using your LinkBlue ID.  

    2. Click on  "Upload."

    Follow the directions to upload your immunization documentation, medical records, lab results, etc. Some browsers work better than others; if you are using Internet Explorer and having problems, try using Google Chrome or Foxfire.

    3.  Select the type of document you wish to enter.  You will be choosing from the following:  

    4.   Then click the "Select File" button, select your file and hit "Upload".  

    5.   You can view the documents you have uploaded by clicking the "Documents Already on File" button.  

    6.   A nurse will enter immunizations within 1 to 2 weeks of receiving your information. After 1-2 weeks, log on and click "Immunizations" then "print". If you are missing any requirements, they will be listed under "Missing Requirements."

    7.  You may access the Student Health Portal after your LinkBlue ID becomes active. If you have trouble accessing the portal and your LinkBlue ID is active, please email
