Polk-Dalton Clinic

The UK Polk-Dalton Clinic was named in honor of two physicians from Georgetown, Dr. John Polk and Dr. Rufus Dalton, both of whom helped blaze a trail to deliver health care in downtown Lexington.

The clinic's priority is providing health care with compassion for Lexington's Northside and urban community. It offers convenient family medical services for the entire family, including infants, school-age children, teens, adults, parents and grandparents.

Your First Visit

On your first visit, please arrive 30 minutes before your appointment time to complete registration forms. If your first visit to the clinic is on a walk-in basis, you will also need to complete registration forms.

Please bring all of your medications or a list of medications you are currently taking. For new pediatric patients coming in for a physical, please bring a record of immunizations. A thorough medical history helps us provide you with the best possible treatment.

Payment

Polk-Dalton Clinic accepts most insurance plans, including Medicare and Medicaid. When presenting a medical or insurance card, a picture I.D. must accompany it. Payment arrangements can be made for uninsured patients.

Cancellations

If you must cancel your appointment, we ask that you give us 24 hours notice so we can see another patient at that time and arrange another appointment for you.
