The Phyllis D. Corbitt Community Health Center is a nurse-led primary care clinic in Wilmore, Ky., that is an extension of UK HealthCare. Anthony Carney, DNP, APRN is a family nurse practitioner and a Transform Health provider. He sees patients on Mondays from 8 a.m. – 4 p.m. and can provide all of the services listed below.

Our services
- Primary care (including chronic care for health conditions, basic mental health care, cancer screenings, screening and treatment for sexually transmitted infections)
- PrEP (for HIV prevention)
- Hormone therapy for adults
- Gender-affirming, family-centered general care and referrals for children and adolescents*
- Mental health care referrals (both in and out of UK)
- Surgery referrals (both in and out of UK)
*UK providers practice in accordance with Kentucky law. We cannot, therefore, provide puberty blocking or hormones to those under 18, though we are happy to continue with primary care, chronic care, and referrals for our transgender spectrum patients of all ages.
