Local Clients

Please download and complete the Service Request Form hyperlinked under each category of service and bring it with your sample to HA 629, sixth floor, Chandler Hospital. Please also email the electronic version of the Service Request Form and any associated sample information documents to  genomics@uky.edu.

Long-Distance Clients

Please download and complete the Service Request Form hyperlinked under each category of service and enclose a printed copy with your shipped samples. Please also email the electronic version of Service Request Form and any associated sample information documents to genomics@uky.edu.

All Clients

Please download and complete the UK HealthCare Account Request Form (link below) if you have not already established an account with UK HealthCare. Email the completed form to genomics@uky.edu. 

Please note that each account is specific to one cost center/WBS element. For charging to multiple grants, you should request a UK Healthcare billing account for each grant.
