  • All college transcripts (unofficial or official).
  • Statement or letter of eligibility for internship from academic director, sent directly from academic director to the internship director. Please ask your academic director to email the letter of eligibility to Katrina Koszczuk at katrina.koszczuk@uky.edu.
  • Two (2) signed letters of recommendation, sent directly from the writers to the internship director. Please ask your recommenders to email their letters to Katrina Koszczuk at katrina.koszczuk@uky.edu.
