Quality Awards
In 2025 North Fork Valley Community Health Center and June Buchanan Medical Clinic earned five Health Resources and Services Administration (HRSA) quality awards. HRSA programs provide health care to people who are geographically isolated and economically or medically vulnerable.
Of the roughly 1,500 similar health centers in the nation, North Fork Valley and June Buchanan were in the top 10% of programs to receive the National Quality Leader in Diabetes Health award and in the top 12% to receive the Behavioral Health award.
They were in the top 16% to receive the National Quality Leader in Heart Health award and in the top 8% to receive the Preventive Health award.
North Fork Valley and June Buchanan were also once again recognized as a Gold Health Center Quality Leader for overall quality measures, placing them in the top 10% of all health centers.
