  • FAA Pilot & Air Traffic Controller Exams

    AME Examiners: Dr. T. Scott Prince is a certified Aviation Medical Examiner with over 20 years of experience. He performs FAA Class I, II, II, and student exams as well as Air Traffic Controller Exams at the UK HealthCare Turfland facility.

    FAA Medical Application: Submit your FAA Medical Application before your appointment with our AME doctors. A confirmation number will be provided after a successful application has been processed. Please bring that number with you to your appointment. Find the application here.

    Payments: All payments for the FAA and ATC exams are due upon checkout.

    How early should I come for my appointment? We ask our patients to arrive 15 minutes before their appointment at the UK HealthCare facility at Turfland, 2195 Harrodsburg Road, to fill out the registration form (PDF at link below).   • DOT and CDL Exams

    Certified examiners: Dr. T. Scott Prince and Dr. Susan E. Spengler are certified Department of Transportation examiners with more than 50 years of combined experience. Further, our doctors are certified and listed in the National Registry of Certified Medical Examiners.Our nurse, Paula, can perform NIDA and non-NIDA Drug Screens as well as breath alcohol tests.

  • USCIS Immigration Medical Examinations

    USCIS Civil Surgeons: Dr. Susan Spengler is a designated United States Citizenship and Immigration Services Civil Surgeon and provides medical examinations for applicants who need adjustment of status. In ideal situations, our Civil Surgeon can complete this process in two days.

    Form I-693: The purpose of Form I-693 is to report results of a medical examination to USCIS. The examination, performed by a Civil Surgeon, is required to establish that an applicant is not inadmissible to the United States on public health grounds. After completion of the medical examination, our civil surgeons will give you the completed form in a sealed envelope for you to file with the USCIS.

    What do I need to bring to my appointment? We ask our patients bring any and all past immunization records as well as a government form of photo identification (i.e. passport or driver's license). 

    What should I expect at my appointment? A Form I-693 medical examination consist of three things. First, a TB skin test which checks for Tuberculosis. This test has to be read 48-72 hours after administered. For example, a TB skin test given on Monday will have to be read on Wednesday. Second, a blood draw will be performed to identify syphilis. Lastly, the Civil Surgeon will perform a medical examination as well as identify any immunizations you may need to complete the form.

    How early should I come for my appointment? We ask our patients to arrive twenty minutes prior to their appointment to fill out the registration form, medical history, and Form I-693. These forms are also provided at the links below. If possible, please print them, fill them out and bring them with you to your appointment.

    Will insurance cover my clinic visit? All payments for the medical examination are due upon checkout.

    • Adult medical exam (15 and older), $225
    • Child medical exam (14 and younger), $200
    • Repeat exam (initial exam done at UK), $200
