Facts about obtaining your medical records
Medical records are official records of UK HealthCare activity and are considered UK property. You have the right to obtain a copy of your medical records, but in order to protect your privacy, we must have your written permission before releasing the records.

You may download a copy of the UK Release of Information Form, then fill it out and mail or fax it to the Medical Records Department (address at bottom of page). You may also hand deliver your form to the Health Information Library located in Room C104 near the Chandler Hospital lobby. Instructions are available to help you complete the form, or library staff is also available to assist you in submitting your request. Incomplete forms may be returned to you for completion. By law, no information can be released unless the form is properly signed. Please specify on your request which medical records you want.

In order to view or print the form, you must have a copy of Adobe's Acrobat® software. Go to Adobe's Web Site to download a free copy of Adobe Acrobat Reader.

If you cannot download the release form, please call 859-323-5117 and a copy will be sent to you.

Costs
The first copy of your records is free. A standard fee of $1.00 per page will be charged for additional copies. Please keep a personal copy of any information you request to avoid future costs of obtaining copies.

At your request, your medical records will be released to any doctor free of charge. Call 859-323-5117 for more information.

When will I get my records?
Requests are usually completed within 30 days. Records will be mailed to the address listed on the UK Release of Information form. If you would like to pick up your records, make a note on the form with a phone number where you can be reached.

Where do I submit requests?
Services are provided at many different locations and facilities at UK HealthCare. Each location is responsible for keeping and releasing its own medical records. The contact mailing address for each location is at the bottom of the Release of Information form.

Send your request to the location where you received your care to reduce processing time.

Hospital medical records
If you want to see your records while you are in the hospital, you may request a copy from the UK Chandler Hospital Medical Records Department by calling 859-323-5117.

The UK Chandler Hospital medical records office is open Monday through Friday from 8 a.m. to 4:30 p.m.

Kentucky Clinic medical records
The Kentucky Clinic has a central medical records office located in the basement of Kentucky Clinic in Room K-003. The office is open Monday through Friday from 8 a.m. to 5 p.m. This office can assist with any requests for outpatient medical records from any of the clinics located at the main Kentucky Clinic attached to the hospital.

The Kentucky Clinic medical records office can be reached by calling 859-323-5561. Medical records requests can be faxed to 859-257-7228.
