PharmacistCARE helps employees manage diabetes, cardiovascular risks
PharmacistCARE is a medication therapy management service that offers two unique programs—DiabetesCARE and CardioCARE—to help patients with diabetes or risk factors for cardiovascular diseases such as high blood pressure or cholesterol control their diseases and lead healthier lives.

Participation in either the DiabetesCARE or CardioCARE program is free of charge to all adult UK Health Plan members and includes:

• Group educational classes and one-on-one counseling.
• A quarterly newsletter and continually updated educational materials.
• A distance education program for patients who live outside the Lexington service area.
• Telephone case management for CardioCARE participants through the UK Behavioral Health Improvement Program (BeHIP).
• A reduced co-payment for test strips for DiabetesCARE participants ($10/mo. formulary, $30/mo. non-formulary).
