Division of Pulmonology
UK Pediatric Pulmonology
includes two pulmonologists, a pulmonary nurse-specialist, a pulmonary
function technician/research assistant, a respiratory therapist, a dietitian,
clinical psychologist and an administrative assistant. Clinical services
of the division include the cystic fibrosis clinic, the infant monitoring
program and the pediatric pulmonary function and sleep laboratory.
More than 175 patients with cystic fibrosis from infants to adults
are followed by the CF team. Cystic fibrosis clinic is held once a week,
and an average of 20 patients are seen in each clinic. In addition to
the division personnel, the clinic is attended by pediatric residents
and medical students. This clinic provides residents an opportunity
to understand the broad spectrum of disease severity, especially those
patients with minimal symptoms who are rarely hospitalized. The CF center
is involved in exciting research projects involving new therapies that
are becoming available.
The Pulmonary Clinic follows children with acute and chronic respiratory
problems such as asthma, bronchopulmonary dysplasia, and congenital
anomalies of the lung and bronchiectasis. Infants on a home apnea monitor
or home mechanical ventilation also are evaluated and followed in pulmonary
clinic. The new referrals provide the residents with first-hand experience
in evaluation and management of respiratory problems seen in the community
that warrant consultation.
Pulmonary procedures including fiber optic bronchoscopy and laryngoscopy,
thoracentesis, chest tube placement and transthoracic lung biopsy are
performed routinely. A state-of-the-art endoscopy unit has improved
both service and teaching during bronchoscopic procedures. All procedures
routinely are done with videoscopes, allowing observers to watch the
procedure on a television screen.
Pediatric pulmonary function tests are available both in the clinic
and on an inpatient basis. Plans to develop an infant pulmonary function
laboratory as well as regional clinics are underway. Infant sleep studies
and pneumograms are done under the supervision of the division faculty.
Residents have the opportunity to learn to interpret these studies.
Teaching activities are an important component of the division. Residents
in all services have close contact with the pulmonary team on a regular
basis. Pulmonary patients are admitted to the pediatric ward team. Pulmonary
consultations in both the pediatric and neonatal ICUs are a regular
part of the pulmonary service. During this pulmonary elective, residents
have the opportunity to participate in all clinical, teaching and research
activities of the division faculty. In addition, residents are encouraged
to participate in a project pertaining to pediatric pulmonology and
present it to their colleagues at noon conference.
