Your visit to UK Albert B. Chandler Hospital
Once you arrive at UK A.B Chandler Hospital, please come to the Admitting Department. Admitting is located on the first floor of the main hospital, through the revolving door. The information desk at the main entrance is available to assist you. After you register, you will be shown to a room.
When your physician determines that you are ready to leave the hospital, a discharge order will be written. Your medical team will talk with you about your future health care needs. You may want to make arrangements with a family member or friend to help you when it is time to go home. For assistance with discharge planning, please call Patient and Family Services at 859-323-5501, 8 a.m. to 5 p.m., Monday through Friday. When leaving, be sure you understand the following:
- Your medications - ask about our Meds-to-Beds Program.
- Your plans for follow-up care.
- Who you should call if you have a question or problem.
- Recommended diet and any activity restrictions.
Collect all of your belongings and double-check closets and drawers. If you have anything stored in the hospital safe, please call Hospital Security at 859-323-6946, Monday through Friday, 8 a.m. to 5 p.m., to claim your items. If you want late-arriving mail and gifts to be sent to an address other than the one provided when admitted, please contact the Volunteer Office at 859-323-6023.
Assistance is provided to patients and their families to find transportation to another health care facility or to their homes. Call Patient and Family Services at 859-323-5501, Monday through Friday, 8 a.m. to 5 p.m.
Most follow-up care is provided at the Kentucky Clinic, located across the street from the hospital. For more information or to schedule an appointment at the Kentucky Clinic, call 859-323-5000, Monday through Friday, 8 a.m. to 5 p.m.

Completing a patient satisfaction survey
Patients receive a satisfaction survey in the mail. Patients or their designees are encouraged to complete the survey and return it in the pre-paid envelope. The surveys are used to identify unmet patient needs, improve care and recognize team members who have provided outstanding service.
