This convenient option for busy and active women is available to healthy women between the ages of 40-64 who have no active health disorders.

All in one Friday morning appointment:

• Visit with a physician specializing in women’s health
• Annual exam and pap smear
• Laboratory tests as needed (blood/urine analysis)
• Mammogram screening
• DEXA bone density exam/interpretation (for women more than 50 years of age

Download this form, print it out, then fill it out and fax or mail it to the clinic at the address on the form.  Once the questionnaire is received, our receptionist will call to schedule your appointment.
